I don't know of a way to do this though I haven't used main menu in a long time.
There is a program over on XDA-Devlopers called Touch Launcher that will give you what you want though. I'm pretty sure it is not being developed anymore but it gives a finger scrollable launcher that you can also skin. Since it has a start menu icon it also can be slaved to a hard button on your device. I used it in my 6.1 ROMs and it worked very well.

For 2) all of these will give you a shortcut in the \windows\start menu\Programs folder, so you will be able to map them to any key or use the SoftKeyApplet.cpl to remap soft keys.
Some of these launchers do replace the functionality windows start button (eg launch Manager), but I am not sure that they all do.
